個人情報の取り扱いに関する基本方針

当病院は、人びとの人権、プライバシーを最大限尊重する立場から、個人情報を適正、正確かつ安全に取り扱い、不当な流出、目的外利用から保護することを社会的使命と考え、以下の宣言をします。

  1. 当病院は、個人情報の取り扱いについて職員が遵守しなければならない規定を策定します。この規程は、広く公衆に公開し、その理解が得られるように努めます。
  2. 当病院は、個人情報の収集・利用・提供について、該当個人の固有の権利(個人情報自己コントロール権)を尊重し、收集目的・使用範囲の限定を明示し、適切かつ安全な管理に努めます。
  3. 当病院は、個人情報の改ざん、破壊、紛失、漏洩などが発生しないように、最大限の安全対策を実施します。
  4. 当病院は、個人情報に関する法令その他の規範等を遵守します。
  5. 当病院は、個人情報保護の重要性について、職員に対して教育啓発活動を実施するほか、個人情報保護の管理責任者を定め、適切な管理・監督体制を確立するとともに、苦情への対応を行う窓口を設置します。
  6. 当病院は、以上の活動について絶えず内容を見直し、改善に努めていきます。

患者さん・利用者様の個人情報の取り扱いについて

  1. 1. 当病院は、患者さん・利用者様の医療・介護サービスを最大限効果的に実施するという目的で、患者さん・利用者様の個人情報を収集し、記録し、保管し、活用させていただきますのでご了承ください。 なお、患者さんの医療・介護サービスに必要な個人情報の収集・記録・保管・活用するにあたっては、サービスの提供を申し込まれた時点で包括的に患者さんの同意が得られたものとして取り扱いますが、 その同意を取り消したり一部修正したりできますのでその際は関係職員に申し出てください。
    1. (1) 個人情報の内容の例示は次の通りです。
      1. 患者さん、利用者様の主観的な訴え、これまでの病歴、家族構成など
      2. 患者さん、利用者様の身体に関する客観的な測定結果、撮影された画像、それらに関する従事者の所見など
      3. 診療・介護サービス提供の経過
      4. 各診療・介護サービス従事者の患者さん、利用者様の状態に関する所見
      5. 医療費・介護費の支払いに必要な個人情報(保険番号、住所等)
    2. (2)個人情報の当病院内での使用の例示は次の通りです。
      1. 診療・介護サービス提供を効果的にすすめるための判断材料
      2. 法令に基づく医療費の請求事務
      3. 当病院が受け入れている学生の実習、当病院以外の医療機関、介護機関、消防関係機関等に属する研修生の研修。
      4. 病棟の管理・外来窓口の管理
      5. 法令に基づく公的機関の監査・監視
      6. 病院の診療及びその他の業務の向上に資する目的で、個人情報には該当しない症例資料、副作用・事故資料、統計資料等を作成する場合
      7. 病院からのお知らせ等をお送りする場合
    3. (3)個人情報の第三者への提供の例示は次の通りです。
      1. 当病院以外の医療機関、介護サービス機関と連携して患者さん、利用者様にサービスを提供する場合
      2. よりよい診療・介護サービス提供のため、当院以外の医療・介護機関の医師、薬剤師等専門家の意見・助言等を得ようとする場合
      3. 検体検査など特定の専門的業務を当院以外の機関に委託する場合
      4. 医療事務・電子計算機業務などを当院以外の機関に委託する場合
      5. 法令に基づいて、公の機関に対する申請・届け出・通知を行なう場合
      6. 専門医・認定医など従事者の資格認定を申請する場合
  2. 2. 患者さん・利用者様本人は、自らの個人情報の収集・使用のあり方について、自らの意見を表明する権利があり、私たちはその意見を尊重します。
    1. 特定の第三者(家族も含む)からの情報収集を拒否することができます。
    2. 特定の第三者(家族も含む)への情報提供を拒否することができます。ただし、法令等で医療・介護機関の義務とされる場合は例外とします。
    3. 特定の個人情報の利用(研究・研修その他、直接本人の診療・介護サービス提供とは関係ないもの)を拒否することができます。
    4. 個人情報の収集・使用のあり方について説明を求めることができます。
    5. 次のような個人情報の取り扱いを求めることができます。
      1. ①氏名を匿名化して表示したり呼び出したりすること。
      2. ②病室入口に氏名を表示しないこと。
      3. ③入院の有無・病室等の情報に関する問い合わせに病院が応じないこと。
  3. 3. 患者さん・利用者様の個人情報は基本的に患者さん・利用者様のものであり、情報の開示、誤りの訂正、消去等を当院に求める事ができます。
  4. 4. 当院では、個人情報管理責任者を定め、管理・監督の体制を確立し、個人情報の漏洩、破壊、改ざんを阻止するための最大限の安全対策を講ずるとともに、患者さん・利用者様の苦情に対応する窓口を設けます。皆様のご意見等をお寄せ下さい。

With regard to the collection, use, and provision of personal information, the hospital respects the inherent rights of the individual concerned (the right to self-control of personal information), clearly indicates the purpose of collection and the limitation of the scope of use, and strives for appropriate and safe management.
  3. Our hospital will implement maximum safety measures to prevent falsification, destruction, loss, leakage, etc. of personal information.
  4. The hospital will comply with laws, regulations, and other norms regarding personal information.
  5. In addition to conducting educational and enlightenment activities for staff on the importance of protecting personal information, the hospital will appoint a person responsible for the protection of personal information, establish an appropriate management and supervision system, and establish a contact point to respond to complaints.
  6. Our hospital will constantly review the contents of the above activities and strive to improve them.

Handling of Personal Information of Patients and Users

  1. 1. Please note that the hospital collects, records, stores, and uses the personal information of patients and users for the purpose of providing medical and nursing care services for patients and users as effectively as possible. When collecting, recording, storing, and utilizing personal information necessary for medical and nursing care services of patients, it will be treated as if the consent of the patient has been obtained comprehensively at the time of application for the service, but the consent can be revoked or partially modified, so please notify the relevant staff in that case.
    1. (1) Examples of the contents of personal information are as follows.
      1. Subjective complaints of patients and users, medical history, family structure, etc.
      2. Objective measurement results of patients and users' bodies, images taken, and workers' observations related to them, etc.
      3. Progress in the provision of medical and nursing care services
      4. Findings on the condition of patients and users of each medical and nursing care service worker
      5. Personal information required for payment of medical and nursing care expenses (insurance number, address, etc.)
    2. (2) Examples of the use of personal information in our hospital are as follows.
      1. Judgment materials for effectively promoting the provision of medical and nursing care services
      2. Billing of medical expenses based on laws and regulations
      3. Practical training of students accepted by the hospital, and training of trainees belonging to medical institutions, nursing care organizations, firefighting-related organizations, etc. other than our hospital.
      4. Management of wards and outpatient desks
      5. Auditing and monitoring of public institutions in accordance with laws and regulations
      6. For the purpose of contributing to the improvement of hospital medical care and other operations, when creating case data, side effect / accident materials, statistical data, etc. that do not fall under personal information
      7. When sending notices, etc. from hospitals
    3. (3) Examples of provision of personal information to third parties are as follows.
      1. When providing services to patients and users in cooperation with medical institutions and long-term care service organizations other than our hospital
      2. When seeking to obtain the opinions and advice of doctors, pharmacists, and other specialists of medical and long-term care institutions other than our hospital in order to provide better medical and nursing care services
      3. When entrusting specific specialized work such as in vitro testing to an institution other than our hospital

  3. 3. The personal information of patients and users basically belongs to patients and users, and you can request us to disclose information, correct errors, delete etc.

We will appoint a person responsible for personal information management, establish a management and supervision system, take maximum safety measures to prevent leakage, destruction, and falsification of personal information, and establish a contact point to respond to complaints from patients and users.
